3. Additional practice members

The $1,000 annual practice membership includes the primary account holder plus four additional applicants (five total). For each additional person beyond five total, the annual cost increases by $150.

Complete the same professional profile for each member. No separate emails or signature links are required.

I support the mission of the Tucson Aesthetics Advisory Board and agree to uphold its values of patient safety, professional collaboration, ethical practice, and continuing education.

I understand that TAAB is a non-regulatory professional organization and that membership does not constitute certification, endorsement, or licensure.

As the primary practice account holder, I confirm that I am authorized to submit the professional information for all additional members listed on this application, that I have informed them of TAAB's mission and membership expectations, and that the information provided is accurate to the best of my knowledge.
