3. Additional practice members

The $1,000 annual practice membership includes the primary account holder plus four additional applicants (five total). For each additional person beyond five total, the annual cost increases by $150.

Complete the same professional profile for each member. No separate emails or signature links are required.

I understand that TAAB is a non-regulatory professional organization and that membership does not constitute certification, endorsement, or licensure.

Please do not submit patient information, government identification numbers, payment credentials, or other unnecessary sensitive information.

After successful submission, you will receive your personalized annual payment instructions. Membership becomes active only after review and payment verification.
